Squamous cell carcinoma and sarcomatoid proliferation are two subtypes of spindle cell squamous carcinoma (SCSCC). It is still uncommon in the larynx, accounting for only 1% of all SCSCC in the head and neck. This tumour continues to be difficult to diagnose and treat. By describing three cases of laryngeal SCSCC diagnosed in our department and doing a literature analysis, the goal of this study is to update epidemiological, clinicopathological, therapeutic, and prognostic aspects of this rare entity. The patients were all smokers, with ages ranging from 50 to 66. The most common symptom was dysphonia. In every case, the tumour affects the vocal cord. Laryngectomy and bilateral neck dissection were performed on the patient. In all cases, histological examination confirmed the diagnosis of SCSCC. Basaloid squamous cell carcinoma with sarcomatous transformation and osteosarcoma was found in one tumour. One patient's immunohistochemical investigation revealed that he was positive for vimentin, SMA, PS100, and EMA focally, but negative for keratin and P63. One case had lymph node metastases, and the patient was scheduled for adjuvant radiotherapy. After therapy, all patients were effectively managed, and there were no recurrences during follow-up.
