Serum C - Reactive Protein as an Aid to the Diagnosis of Acute Appendicitis and Its Importance in Reducing the Rate of Negative Appendectomy | Chapter 17 | Issues and Developments in Medicine and Medical Research Vol.10

 Introduction: Acute appendicitis is one of the most prevalent abdominal surgical emergencies, and appendectomy, as the only treatment option, is one of the most common surgeries, with a high rate of negative exploration.

The primary goal of this study was to compare the accuracy and efficacy of CRP versus TLC in diagnosing acute appendicitis.
Methods: A total of 200 patients were included in the trial, all of them were diagnosed with acute appendicitis based on a WBC count and clinical examination and were scheduled for appendectomy. Before the operation, these patients' serum CRP levels were calculated together with their WBC counts, but this information was not used in the appendectomy decision-making process. All of the appendixes that were excised were sent for histological testing.
Sensitivity, specificity, positive predictive value, negative predictive value, and diagnostic accuracy of serum CRP were 94.1 percent, 73.3 percent, 95.23 percent, 68.75 percent, and 91 percent, respectively, whereas WBC were 88.23 percent, 60 percent, 92.59 percent, 47.36 percent, and 84 percent.
To lower the rate of negative appendectomy, serum CRP should be used in conjunction with other laboratory markers and clinical examination in the diagnosis of acute appendicitis.
