Ischaemic Strictures of the Jejunum | Chapter 05 | Issues and Developments in Medicine and Medical Research Vol. 8

 Postoperative adhesions and hernias are the most common causes of mechanical small intestinal blockage. The ileum and ileocaecal junction are more common blockage and stricture sites in the small bowel, while the jejunum is an uncommon site for obstruction and strictures. Small intestinal obstruction can also be caused by illnesses of the small intestine wall, as well as benign (inflammatory, ischemic post-traumatic or post-irradiation) or malignant strictures. Intussusception, gallstones, foreign bodies, and other processes that induce intraluminal obstruction, such as intussusception, gallstones, and foreign bodies, all play a role. Ischaemic enteritis is a rare cause, accounting for just around 0.1 percent of cases. Ischaemic strictures of the small intestine are caused by a lack of blood supply to the gut. To be controlled, they require surgical intervention.

The primary goal of the study was to determine the importance of specific clinical and investigative modalities in the diagnosis and management of ischemic jejunal strictures, which are uncommon causes of intestinal blockage like the one described in this case.

Methods: A 40-year-old diabetic woman with a 2-month history of upper GI blockage was presented by the author. Three years ago, she had a stroke, a myocardial infarction, and a temporary bout of intestinal blockage, according to her medical records. An occlusive distal jejunal stricture was discovered during her evaluation. The author scoured the literature for numerous management options before settling on the ideal one for her.

The jejunal segment was resected and an end-to-end anastomosis was performed as part of an exploratory laparotomy. The histology of the section indicated ischemic enteritis with significant vascular blockage, resulting in a stricture. The patient's recovery was uneventful, and she has been symptom-free for the past two years.

In its chronic phase, ischaemic enteritis causes minor intestinal blockage due to intestinal stricture. Diagnostic delay can be caused by differential diagnoses, as well as missing the brief phase of early ischemia that leads to diagnosis.
