The Effect of Mineral Trioxide Aggregate on the Periapical Tissues after Unintentional Extrusion beyond the Apical Foramen: Case Series | Chapter 14 | Highlights on Medicine and Medical Science Vol. 12

 Introduction: Single-step apexification methods utilising mineral trioxide aggregate (MTA) have been reported as effective treatment alternatives for teeth with an open apex, offering more benefits than other options. The controlled apical placement of MTA, on the other hand, is a difficult process to conduct utilising an orthograde method. In three distinct cases, MTA was unintentionally extruded into periradicular tissues during apexification, as described in this case series.

Three adult patients with open apices on their maxillary incisors presented to the Department of Conservative Dentistry and Endodontics for treatment. One-step MTA apexification was done after isolation, standard access, cleaning, and shaping processes. In all three cases, a significant amount of MTA was found extruded on later radiographs.

The teeth were found to be asymptomatic during follow-up examinations in all cases, and radiographs revealed healing of the periapical lesion.

Conclusion: The findings imply that extrusion of MTA into the periapical tissues has no negative consequences, which could be attributed to MTA's biologic features. In any therapeutic context, the present case series emphasises the significance of careful placement of MTA apical plugs and does not propose intentionally overfilling MTA into the periapical lesion.
