Study of Fibroepithelial Lesions of Breast at a tertiary Care Centre- an Audit, Clinicopathological APPROACH and Comparison with WHO Grading | Chapter 14 | New Frontiers in Medicine and Medical Research Vol. 1

 Background: From indolent fibroadenoma to the potentially lethal phyllodes tumour, mammary fibroepithelial lesions include a wide range of cancers (PT). The morphological classification criteria are often difficult to apply, and there is no consensus on what constitutes a suitable resection margin. The current study's goals were to look into the histopathological spectrum of breast fibroepithelial lesions in a tertiary care setting, stratify and classify various fibroepithelial lesions into fibroadenomas and PT, and reclassify all confirmed cases of PT seen during the study period using standard histopathological WHO criteria.


Methods: Records and slides of fibroepithelial lesions of the breast received at the department were retrieved and reviewed between January 2016 and August 2019.

891 fibroepithelial lesions of the breast were found throughout this time. There were 826 cases of fibroadenoma and its variants (92.7%), 34 cases of fibroadenomatoid mastopathy (3.8%), and 31 cases of PT (3.5%). Eight percent (25.8%) of all PT were borderline, four percent (12.9%) were malignant, and the rest (61.3%) were benign.

Conclusions: Breast fibroepithelial tumours are a heterogeneous group of diseases ranging in severity from benign fibroadenoma to malignant PT. The histologic features of several subtypes overlap, and change and progression to a more malignant phenotype are possible. Because there are significant clinical differences between subtypes, accurate pathologic classification is essential for effective therapy. Although some immunohistochemical markers may aid in this differential diagnosis, histomorphology remains the gold standard.
