Surgeons must ensure that all medical records are accurate, thorough, legible, and current, according to the Royal College of Surgeons' guidance, Good Clinical Practice, which was updated in 2014. Improving the quality of operation notes enhances communication among healthcare personnel and aids in the prevention of errors and lawsuits.
