Background: Thyroid gland secondary cancers are a rare occurrence in clinical practise. In addition, colorectal metastasis to the thyroid (CMT) is an extremely rarer occurrence. The primary tumor's origins follow demographic and ethnical patterns, reflecting the most common cancers in various communities. One of the most common types of cancer is colorectal cancer. Despite the fact that cancer is common all over the world, CMT is rarely identified. The majority of them are discovered during the course of treatment for gastrointestinal primary cancers. Survival and, as a result, identification of metastatic disease has become more common as imaging methods, oncological treatment, and follow-up have improved. Due to the paucity of information and the challenges in conducting clinical trials and research, these facts make this entity a diagnostic and therapeutic challenge. Case Summary: This is a case report of a patient who was diagnosed with a CMT of rectum adenocarcinoma four years after neoadjuvant chemoradiotherapy, subsequent curative surgical resection of the main tumour, and intercurrent lung bilateral metastases. Conclusions: Thyroid metastases of extrathyroid origin are infrequent, and extremely rarer when CMT is taken into account. The diagnostic method, as well as oncologic patient survival, is improving, and as a result, the frequency of thyroid gland metastases is increasing. Conclusions: Thyroid metastases from outside the thyroid are uncommon, and much rarer when CMT is taken into account. The diagnostic approach, as well as oncologic patient survival, are improving, and as a result, thyroid gland metastases are becoming more common.
