Hip Muscle Strength and Functional Deficits in Patients with Avascular Necrosis of Femoral Head and Hip Replacement | Chapter 11 | Research Highlights in Disease and Health Research Vol. 3

 Avascular loss (AVN) of femoral head causes pain, joint stiffness, muscle dwindling and physical disability, when in fact hip replacement, usually done for AVN reduces pain, improves flexibility, strength and physical function. This study proposed to assess the substance and functional deficits on account of AVN preoperatively and postoperatively after hip substitute.This was an observational study. Hip muscle substance of all subjects accompanying AVN of femoral head scheduled for hip substitute was taken by modified sphygmomanometer. Self stated Harris hip score and TUG test were also captured for these subjects. All cases underwent physiotherapy during clinic stay. Again all outcome measures were taken post modern replacement on on the era of discharge. Healthy adults were inducted for measuring hip power strength.In this study, 32 subjects, 16 accompanying AVN and 16 healthy adults were inducted. 28-males and 8-women. Mean age-36.88 years in both groups. Isometric power strength preoperatively in AVN subjects of new flexors-76.75±50.67 mm of Hg, abductors-54.56±30.74 and extensor apparatus 75.38±37.32 was considerably reduced (p<0.001) compared to athletic adults,(hip flexors-188.42±15.69, abductors 112.67±21.66 and extensor device-171.71±25.60). Postoperatively strength of hip flexors-93.12±52.58, abductors-63.62±33.74 and extensor machinery-88.94±41.22 was reduced distinguished to healthy adults. Postop important improvement (p<0.05) seen at discharge in substance compared to preoperative values.Conclusion: Muscle substance of hip powers, both in subjects accompanying AVN preoperatively and post replacement, was significantly decreased compared to normals. Postoperatively improvement was visualized at discharge in strength  distinguished to preoperative values.
