We present the first case of primary dengue fever in a child with acute transverse myelitis. An 8-month-old girl arrived with hepatomegaly and acute febrile encephalopathy. Thrombocytopenia, positive IgM for dengue, and normal liver echotexture on ultrasonography were discovered during the investigation. She developed paraplegia during her hospital stay, with no evidence of aberrant hemostasis or plasma leakage. At the T8-T12 spinal level, an MRI revealed symptoms of acute tranverse myelitis. This is the first case of primary dengue fever in a newborn with acute tranverse myelitis. Pathophysiological mechanisms include autoimmune demyelination and direct viral invasion.
