For swellings in the nasopharyngeal and paratracheal regions, a 16-year-old female was referred for a pan endoscopy with multiple biopsies. At the C4 stage, the patient had bilateral tonsil hypertrophy, grade II MPC, two finger mouth opening, and soft tissue swelling compressing the trachea. Following intubation, the patient developed resistance to ventilation and desaturation. The decision was made to remove the tube and hand ventilate the patient. After the tube was removed, it was discovered that the cuff of the tube had herniated towards the upper end of the ETT. The tube was replaced, and the rest of the procedure went smoothly.
