Study on C1-C2 Goel-Harms Fixation: A Free Hand Technique | Chapter 5 | Novel Research Aspects in Medicine and Medical Science Vol. 2

 Short archival description of the evolution of C1-C2 obsession methods, bodily tricks and free hand Goel-Harms fixation method description. A succession of 7 patients were operated utilizing free hand method, X-rays were used only subsequently the positioning of the patient in first position with Mayfield and at the end before locking the sideways rods. The fixation C1-C2 was acted in free hand manner.No snag was documented, in all cases CT scan disclosed correct and safe pressure positioning (Gertzbein grade A). Our purpose was not the follow up of these inmates, because the make inquiries of the patients treated by Goel-Harms was stated in different items before, the main goal is technique writing and its popularization.Using the physical landmarks and with a good preoperative preparation, it is easy for the experienced surgeons to act the technique in a full scope manner, avoiding X-ray uncovering to the patient and the team. This method is safe, reduce considerably the surgical time, the rate of obstacle in this small order is nil, but in spite of is not recommended as usual use for the surgeons accompanying little experience in this place field. The surgeons in the residency program could determine the technique from the seniors earlier practicing it. Understanding of the fundamental subtleties, the surgical duration of C1-C2 free will fixation maybe minimized, as well as ancestry loss, making this type of abscission very practical, effective and reliable. Preoperative anatomic acknowledgment for surgical planning and intraoperative additional calculations and tactile assessment guarantees accurate C1 and C2 free scope screw placement and enough length of the screws. The use of the disciplining of the interarticular space C1-C2 and packing accompanying bone graft from the thorny process of C2 vertebra with subsequent insert of the spacer (screw from beginning cervical plate system, 12-14mm length) embellishes the stability of the obsession and fusion rate.
