In the absence of past disease or surgery, agenesis of the right lobe of the liver is described as the absence of liver tissue on the right side with preservation of the main hepatic vein. The right hepatic lobe agenesis is an uncommon congenital abnormality identified by coincidence during a radiologic test. A 22-year-old lady appeared with abdominal distension and was suspected of having liver cirrhosis, which we discuss. She was examined, and imaging tests revealed that the right lobe of her liver was missing. Agenesis of the right lobe of the liver is a very unusual congenital defect.
