L-carnitine is a cofactor in fatty acid and energy metabolism that has sparked interest in individuals with end-stage renal disease and dialysis. Many variables may predispose individuals with chronic kidney disease (CKD) to carnitine insufficiency; nevertheless, serum carnitine concentrations tend to decline more with longer dialysis duration. For many years, L-carnitine has been utilised as an adjuvant medication in the treatment of HD. In several HD Units, L- Carnitine has been administered inconsistently with an argument and has been found to have few major side effects. As a result, we studied the effects of intravenous L- Carnitine on muscle tiredness and weakness in HD patients. Eleven patients on maintenance HD were enrolled in this clinical and laboratory study at Arar general hospital in the Kingdom of Saudi Arabia (KSA). From November 2017 to February 2020, the observation was carried out for more than 30 months. All of the patients had complained about muscle weariness and weakness. The patients began treatment with an intravenous injection of L-carnitine ampoule (1000 mg) three times per week after each HD session for eighteen months to two years. Due to the inaccessibility of L-carnitine at the start of 2020, all patients will be required to stop using it. Each HD session, the eleven patients in the study received an L-carnitine ampoule, and all of them had improved in terms of muscle weariness and weakness, either completely or partially. Except for one patient, who suffered significantly after the medicine was stopped, the patients complained of muscle weariness and weakness as they had before the start of treatment. Intradialytic hypotension was reported by four of the patients, which went away with the use of an L- carnitine ampoule and reappeared once the L- carnitine was stopped.
Conclusion: In HD patients with specific signs such as intra-dialytic hypotension and muscle weakness or weariness, an intravenous L-carnitine ampoule of 1000 mg three times per week could be beneficial.Author(S) Details
