Excessive use of steroids or alcohol intake can lead to avascular necrosis of the femoral head. Overproduction of endogenous cortisol seldom causes avascular necrosis of the femoral head. The case of a 37-year-old woman with avascular necrosis of the femoral head and adrenal Cushing syndrome is discussed in this study. A cortisol-producing adrenal adenoma was verified by biochemical and pathologic analysis. The patient's ongoing right pelvic pain triggered further testing, which revealed the existence of avascular necrosis of the right femoral head. After a left adrenal resection and complete right hip arthroplasty, the patient was discharged and has been treated without relapse. Avascular necrosis of the femoral head may appear as an early symptom of endogenous cortisol development, according to this case study.
