Chronic myeloid leukaemia (CML) is a liquid tumour that happens due to reciprocal fluctuation between deoxyribonucleic acid 9 and 22 (Philadelphia chromosome) leading to the beginning of a fusion gene bcr-ABL that up- manages tyrosine kinase activity and prevents apoptosis. A 26 years male presented accompanying complaints of sudden beginning non traumatic impulsive swelling over the left scapular domain and progressively extending up-to the abandoned anterior rib cage wall which was guide dull aching moderate force pain. Swelling was warm, soft inconsistency and tender. He also had hepato-splenomegaly. Myeloid hyperplasia and a evident leucocytosis were seen in the overall blood picture. The disease of spontaneous hematoma accompanying CML was confirmed by diagnostic goal after a CT scan of the thorax told a large hypodense injury in the scapular and anterior chest obstruction region. The existing hematoma acted not increase in size and no new draining complication happened later imatinib commencement. Surgical approach in such cases is picked only in presence of difficulties like expanding hematoma, superimposed contamination, abscess or mass effect at lively locations.The hematoma was acted conservatively with medicines while imatinib therapy was initiated.
