Home-based Pediatric Palliative Care Model Effectiveness in the Developing Countries Context: A Retrospective Analysis - Ukraine Experience | Chapter 14 | Recent Developments in Medicine and Medical Research Vol. 6

 Background: There are two types of paediatric palliative care services available around the world: institutional and home-based. It is also used to integrate those models.

The goal of this study is to determine the efficacy of a home-based paediatric palliative care strategy in Ukraine's emerging economy.

Design: Data from paediatric palliative care providers across Ukraine was analysed retrospectively using several paediatric palliative care models.

Sources of Information: The researchers reviewed original research data from January 2014 to December 2018 based on records from the Ukrainian National Scientific Medical Library database for four service providers (N=4) (one institutional, two home-based, and one integrated model). Seven studies (N=7) were assessed based on the following outcomes: morbidity and mortality, health-care use, patient quality of life, cost-and-time-effectiveness, and care continuity.

Results: In Ukraine, the use of a home-based paediatric palliative care approach improves health-related and health-care usage outcomes, as well as patient quality of life, by addressing a broader range of psychological/social demands, including home-based death. It is more cost- and time-effective than the institutional model in developing economies. Because of Ukraine's geographical characteristics, it provides for a larger population coverage and better use of health services. It is critical to develop rapid transfer protocols and ensure continuity of care by coordinating services between different care providers, defining a detailed scope of national health insurance coverage for home care services, and defining a detailed scope of national health insurance coverage for home care services.

Conclusion: The cost- and time-effectiveness of establishing mobile units increases the use of health-care resources in low- and middle-income countries. In low- and middle-income countries, the home-based paediatric palliative care approach is an effective way to build national paediatric palliative care infrastructure.
