Characterizing Pain in Varicocele and the Impact of Varicocelectomy: A Single-Center Prospective Study | Chapter 3 | Medicine and Medical Research: New Perspectives Vol. 6

Introduction: Varicocele is found in up to 15-20% of young males. Although asymptomatic in most cases, it can present with pain, infertility, testicular atrophy, or visible swelling. Despite, some studies supporting the role of varicocelectomy in pain relief, current guidelines do not mention pain as an indication for surgery. Therefore, the application of varicocelectomy in pain relief remains somewhat controversial.

 

Aim: The aim of the study is to analyze the characteristics of pain in patients with varicocele and the outcome of varicocelectomy surgeries on pain perception.

 

Patient and Methods: In a 14-month period, 36 consecutive patients with pain secondary to varicocele were prospectively enrolled in this study and underwent varicocelectomy. History, physical exam, detailed explanation and scoring of pain, scrotal ultrasound, and semen analysis were evaluated before and after surgery. Pain details were assessed through a patient questionnaire which included questions about the type of pain, its location, and exacerbating and relieving factors. The severity of pain was then scored by patients based on an analog numerical scale that corresponded to their level of pain. Varicocele grading was performed based on the World Health Organization (WHO) grading scale for varicoceles. Patients were followed for one year after surgery.

 

Results: Thirty of 36 patients completed the study with appropriate follow-up visits. Patients were 17- to 34-years-old (mean 24.5±4). In 57% of patients, pain was the only presenting symptom. Pain was described as sharp in 11 (36%), burning in 5 (17%), dull in 4 (13%), and vague and hard to explain in 10 patients (33%). Pain most commonly was experienced in scrotum, inguinal, and lower abdominal area. After surgery, 12 patients (40%) had complete relief of pain. Seven patients (23%) had significant relief; seven patients (23%) had moderate relief. Overall, 26 patients (87%) had moderate (more than 50%) to complete pain relief (P<0.005). Four patients had minimal or no improvement in pain after surgery.

 

Conclusion: Pain related to varicocele is mainly experienced in the scrotum, inguinal, and lower abdominal area. Varicocelectomy completely or significantly resolves the pain in up to 86% of patients.

 

Limitations: This prospective study has some limitations, including a small sample size and lack of representation across multiple institutions, both of which limit generalizability. Moreover, the subjective nature of pain makes analyzing the benefit of varicocelectomy less consistent among patients. Finally, the association between varicocele grade and severity of pain was not investigated.

Role of Audio Distraction in Management of Anxiety in Children| Chapter 10| Medicine and Medical Research: New Perspectives Vol. 8

The study aimed to determine the role of audio distraction in the management of anxiety in children. Dental anxiety is one of the most commonly encountered problems in the dental clinic. Anxiety, if not handled in the right way, can compromise the quality of dental care. As a dental clinician, there are various methods of behavior management; one of them is Audio distraction. Music has always been one of the most commonly used methods to relieve anxiety So, this review was written with the intention to study the effect of Audio distraction in the management of anxious dental patients. Audio distraction is one of the most efficient forms of non-invasive behavior management techniques, as the equipment used are musical instruments, headsets, and audio players among others. Various studies have concluded that audio distraction causes a noticeable reduction in the anxiety of the patients. While being an effective tool in behavior management it has not been proven to reduce pain perception in patients. Hence, in the plethora of behavior management techniques, audio distraction can be employed to play a pivotal role.

Ozone Therapy in the Management of Trigeminal Neuralgia | Chapter 12 | Medical Science: Trends and Innovations Vol. 3

Trigeminal neuralgia includes a complex of painful, intensely explosive (electric shock-like) manifestation in the oral and maxillofacial area, sometimes accompanied by sweating or local hypersensitivity. Due to the extremely intense neuropathic pain, Trigeminal neuralgia affects the patient’s quality of life. This study presents a case of a 54-year-old patient with hemicrania and hypersensitivity on the right side of the face, who spoke with his mouth almost closed. After 12 sessions of ozone therapy (subcutaneous infiltrations), the pain disappeared, gradually, evaluated by VAS (Visual Analog Scale) and the other symptoms were significantly improved. Ozone therapy is used in chronic diseases, in treating pain, and for rejuvenation purposes, by injections, insuflations, autohemotransfusion and locally applied. Also, ozone therapy presents disadvantages characterized in rare cases by respiratory or neurological complications. However, ozone therapy remains an alternative treatment modality to classic ones.

The Neural Pathways and Evolutionary Supports for the Nociceptive-Sympathetic Coupling of Pain Sensation Hypothesized by the Athletes of NBA in Television | Chapter 4 | Advancement and New Understanding in Medical Science Vol. 8

It was proposed by the US athletes of National Basketball Association (NBA) in television on May 6, 2016, that the pain sensation might result from the nociceptive-sympathetic coupling after the peripheral nociceptive afferents. In this article, it is supplemented the neural pathways and evolutionary supports for this nociceptive-sympathetic coupling. For the neural circuits of pain sensation, presently it is widely accepted that the Aδ- and C-fibers relay the nociceptive signals to the laminae I, II and V at dorsal spinal horn, and in turn activate the periaqueductal grey (PAG) in midbrain sending inhibitory feedbacks via raphe to the spinal nociceptive transmission. Herein it is supplemented that, besides such feedback pain circuits, the neurons in laminae I and V of dorsal spinal horn and those in PAG can directly activate the sympathetic outputs, completing the nociceptive-sympathetic coupling. It is suggested that such nociceptive-sympathetic coupling partially contribute to the pain sensation because of the sympathetically maintained pain. Furthermore, by the evidence of somite nociceptive coupling in Drosophila, it is additionally supported by evolution the nociceptive-sympathetic coupling for pain sensation at spinal cord in vertebrates. In all, it is delineated the neural pathways and supplemented the evolutionary supports for the nociceptive-sympathetic coupling of pain sensation.

Exploring the Relevant Assessment Procedures in Evaluation of Patients with Knee Osteoarthritis | Chapter 11 | Advancement and New Understanding in Medical Science Vol. 7

The management of knee osteoarthritis (OA) depends on the current state of joint degeneration and its influence on the individual’s functional health status. To ensure a comprehensive understanding that facilitates informed decision-making, it is imperative to integrate radiological assessment with clinical and functional evaluations. Pain assessment should adopt a paradigm that considers both nociceptive and neuropathic sources of pain, considering the potential role of peripheral and central sensitization in pain manifestations and its ramifications on affective and cognitive spheres. Functional assessment should encompass three dimensions, according to the view of the reported results and the degree of objectivity. Additionally, given the notable prevalence of depressive symptoms and anxiety among individuals with knee OA, mental health status must not be overlooked, as it significantly influences functional status and predicts rehabilitation outcomes. Consequently, the results obtained from the functional test should be considered in light of the potential impact of depressive symptoms. However, it is crucial to recognize that depressive symptoms may exert distinct influences on the three functional dimensions, contingent upon the level of objectivity. Regarding pain, a lack of clear consensus exists on its influence, with divergent definitions of pain and the inclusion of heterogeneous cognitive factors related to pain impeding the derivation of conclusive results. Ultimately, a comprehensive assessment incorporating radiological, pain, functional, and psychological statuses is essential to inform optimal treatment decisions tailored to the specific stage of knee OA.

A Case Report on Comparitive Therapeutic Effect of Transdermal Patches of Ketoprofen with that of Fentanyl in Pain Control | Chapter 6 | Current Innovations in Medicine and Medical Science Vol. 5

 Numerous cases won't be relieved of their symptoms after surgery because the remedial efficacity of the medicine varies from case to case. The thing of this case report is to compare the remedial efficacity of two patches, ketoprofen and fentanyl, inpost-operative cases. We present a case report on the extent of pain relief compared with visual analogue scale scores( VAS score) afterpost-operative surgery other than chine surgery using ketoprofen transdermal patch and fentanyl transdermal patch on cases. The subjects were divided into two groups, manly and womanish, and the treatment was given for over to three days.

After completing the study, we concluded that the subjects who entered ketoprofen remedy had the loftiest VAS score when compared to those who entered fentanyl remedy, and complete pain relief was observed in three days. The statistical analysis revealed that it was largely significant.

Quantum Pain Management Concept| Chapter 13 | Current Practice in Medical Science Vol. 10

 Up until today, there hasn't been an objective definition or assessment of pain. The widely accepted definition of pain lacks a number of essential elements, leading to exceptions to the generalisation. Science cannot adequately characterise something that cannot be precisely measured and manipulated. The usual mechanistic search for a specific area of the brain or ascending pain-related route may be to blame. After this tactic failed, fresh concepts were proposed, including the pain matrix and the dynamic pain connectome. Because the pain network is a multifractal and non-linear system, it can be examined utilising complex and chaotic system analysis, quantum mechanics, and quantum information theory. A new perspective on pain based on quantum physics is therefore required.

Identifying Relations between Posture and Pain in Lower Back Pain Patients | Chapter 14 | New Horizons in Medicine and Medical Research Vol. 3

 "The body's attitude or the arrangement of the limbs when standing or sitting" is how posture is defined. There are several other positions to choose from, including lordotic, swayback, flat back, and anterior pelvic tilt. Everyone views pain to be an unpleasant feeling or sensation in other elements of their lives. Low back pain, in particular, can be debilitating and frightening for many people, who can often recover without the help of a doctor or any medication by simply changing their posture or completing the action. A range of factors, in addition to the postural defect, contribute to low back discomfort, including age, sedentary lifestyle, anxiety, and sleep. When it comes to back discomfort, though, posture has become deeply ingrained in people's minds. Back pain can be alleviated by correcting one's posture, according to health professionals. Our posture deteriorates as we age, yet this does not appear to be a source of pain. If you can't move your body, it's not because you have a posture problem; it's because you have a mobility problem. According to a study, there is no variation in the lordotic angle of people who suffer from LBP. The range and speed of the movement done has decreased. It's your mobility, not whether you're standing or sitting, that counts. The study's main purpose was to see if there was a link between static posture and discomfort. Researchers looked into whether there was a link between low back pain and posture in patients with low back pain. According to current research, there is no link between posture and low back pain. This evidence has been backed up by numerous studies. When doing a big deadlift, landing a leap, or engaging in any physical exercise, we allude to the importance of posture and alignment. As a result, the motivation to produce a paper on this subject arises from a desire to change the mindset of many people who are concerned about their postural defects or appearances by offering knowledge about varying their static posture to conform to some ideal and staying active to improve function.

Study on Administration of Placental Extract for Pain and Quality of Life in Patients with Fibromyalgia Syndrome | Chapter 08 | New Frontiers in Medicine and Medical Research Vol. 17

 Fibromyalgia syndrome (FMS) is a persistent widespread pain and tiredness disorder that causes patients' quality of life to deteriorate significantly. In addition to traditional treatment, many with FMS use complementary and alternative medicine therapies. In Korea, placental extract (PE) is used to alleviate weariness, growth retardation, and soreness. However, no attempt has been made to evaluate the impact of PE on the therapy of FMS. The impact of PE administration on pain and life quality in patients with FMS was investigated in this study.

64 patients at the clinic who were being treated for FMS were retrospectively examined using acupuncture point injection (API) with PE. Pain and sleep disturbances were significantly reduced after five weeks of APIs with PE. Patients also had longer productive times, indicating that they were able to continue their daily activities without taking a pain-induced rest after receiving API treatments.

A 46-year-old lady with FMS was treated intravenously once a week for 9 weeks with PE-containing 5 percent glucose solution in a different route of PE delivery. The subsequent intravenous dose of PE dramatically improved her weariness, discomfort, and health-related quality of life. PE's therapy impact coincided with a reduction in weariness. Both PE treatments were beneficial for pain control and fatigue alleviation in FMS, according to the data. PE appears to be a safe and promising therapeutic option for FMS, according to our findings.
