Skeletal Class II Malocclusion with Maxillary Constriction - A Comprehensive Treatment Approach | Chapter 11 | New Horizons in Medicine and Medical Research Vol. 12

As seen in this case report, a complete approach including the use of a fast palatal expansion appliance followed by fixed appliance therapy can be utilised to successfully treat a bilateral maxillary posterior crossbite with a severe skeletal Class II malocclusion throughout the development period. A 14-year-old boy was diagnosed with an Angle's Class II division 1 subdivision malocclusion, which included maxillary constriction, increased overjet, a deep bite, and severe crowding of the maxillary and mandibular incisors. The boy also had an orthognathic maxilla, a retrognathic mandible, and a high mandibular plane angle. The goal was to use an orthopaedic phase to first open up the restricted maxilla to allow for normal mandibular development before moving on to an orthodontic phase to address the tooth size-arch length discrepancy. in the dental arches of the maxilla and mandible. The maxillary and mandibular first premolars were removed and Roth's pre-adjusted edgewise appliance treatment (0.022 x 0.28-inch slot) was strapped up to address the substantial tooth size-arch length disparity in order to treat the bilateral posterior crossbite. Following treatment, the patient's soft tissue profile and dentofacial aesthetics significantly improved along with their self-confidence and self-esteem.
