Reporting Two Rare Cases of Surgical Management of Herniated Intrathoracic Gastric Perforation in Traumatic Diaphragmatic Rupture| Chapter 14 | Current Topics in Medicine and Medical Research Vol. 8

 Gastric perforation through a diaphragmatic breach into the thoracic cavity is rare, but patients are in serious pain when it happens, with mortality reaching 50 percent. In the cardiovascular and thoracic surgery department of the Komfo Anokye Teaching Hospital in Kumasi, Ghana, we present our experiences of two rare and unusual cases of traumatic diaphragmatic rupture from penetrating chest injury associated with herniated intrathoracic gastric perforation over a five-year period from January 2015 to December 2020. Via normal posterolateral thoracotomy, both patients underwent successful surgical repair, with one having prior negative exploratory laparotomy. In this extremely unusual complication of traumatic diaphragmatic rupture, the essence of the paper is to share and address the clinical presentation, diagnostic issues, surgical management and postoperative treatment. There is an exceedingly rare correlation of traumatic diaphragmatic rupture with herniated intrathoracic gastric perforation, associated with high morbidity and mortality. The diagnosis requires a high suspicion index. Also with smaller resource centres, quick resuscitation and early thoracotomy surgery saves lives.
