Tilting of Foldable PCIOL Masquerading as Malignant Glaucoma | Chapter 6 | Advanced Concepts in Medicine and Medical Research Vol. 4

Two women reported to our Glaucoma Clinic with discontents of blurred vision & pain in addition to signs suggestive of malignant glaucoma post phacoemulsification waterfall surgery accompanying foldable PCIOL, operated elsewhere. B-flip through showed posterior segment inside normal limit, so ruled out suprachoroidal haemorrhage. I  acted them first with YAG minor iridotomy (PI) & peripheral anterior hyaloidotomy through minor PI with antiglaucoma cures (AGM) & topical Atropine assuming the case expected of malignant glaucoma. But in follow up, prior chamber (AC) was found expected shallow with individual haptic of foldable IOL in sulcus tilting forward seen in dilated learner under the effect of topical atropine being the reason for causing condition masquerading as diseased glaucoma. Then, the cases were treated by redialing of foldable PCIOLs in bag in two together the cases. Post redialing, there was improvement of keen sight along with reformation of AC accompanying normal depth & intraocular pressure (IOP) was again under control outside AGM.
