Role of a Clinical Pharmacist in Minimizing Medication Errors through Medication Reconciliation and Patient Counseling in the Department of Gastroenterology | Chapter 2 | Novel Aspects on Pharmaceutical Research Vol. 8

 This division highlights about the elementary role of pharmacist, search out help in minimizing the mistakes and to perform medication conciliation, which was proved to be an important form in detecting the medication conflicts and preventing adverse patient effects.  Medication errors are ultimate common causes of adverse drug occurrences in patients. A cure error is some preventable event that grant permission cause or lead to unfit medication use or patient harm while the medication is in the control of the healthcare professional, patient, or services. Such events can be related to healthcare products, professional practice, processes, and systems, containing prescribing, ordering, product describing, communication, wrap, compounding, dispensing, disposal, administration, listening, education, and use. The research was a prospective druggist interventional study conducted over a ending of 8 months at MNR Hospital, Sangareddy, Telangana state after taking everything in mind the inclusion and exclusion tests.The sample size captured was 150 patients and the study population constitute of patients old 18-80 years, admitted in the nursing home during the study ending of six months.Medication reconciliation procedures secondhand throughout patient medical care, discharge, and follow-up procedures raised the accuracy of drug reconciliation, and the care given to hospitalized patients fashioned the transition to peripatetic settings safer. Out of 150 inmates there were 98 (65.33%) male sufferers and 52 (34.67%) female patients. Patients between 18 and 30 age of age were 29(19.33%), betwixt the age 30 and 50 years were 71 (47.33%) and above 50 were 50(33.33%). Pancreatitis was most prevalent accompanying 21% of total prevalence, trailed by CLD and cholelithiasis with 17%, then IBD 16%, PUD and Gastritis 5%, GERD 4% and added diseases 15%.The basic responsibilities of a pharmacist search out do medication conciliation and to assist in decreasing wrongs. In order to improve patient safety and management changes for a better outcome, pharmacists acquaint patients about the disease and the drugs they are taking. It is told that the discrepancies were checked and settled by which the patient's character of life was improved and the conflicts were reduced. As most of conflicts were accepted by the specialist, it helped to resolve the wrongs.
