Introduction: Since time immemorial, the class II skeletal jaw difference has posed an enigma to the orthodontists who practice it. This case report explains the treatment of a skeletal Class II malocclusion with an orthognathic maxilla and retrognathic mandible with severely proclined maxillary incisors, imbricated mandibular incisors, deep bite, convex profile and horizontal mandibular growth pattern handled with the Herbst appliance.
Methods: The purpose of visual therapy was initially carried out to confirm the
feasibility of the use of a practical unit. The procedure included the use of a
fixed functional appliance to advance the mandible forward, namely the Herbst
appliance. At T1 and T2 of the initial orthopedic process of treatment, the
case was evaluated.
Results: At T2, the mandible retrognathism was sufficiently treated and the
convexity of the face decreased significantly. With the removal of the lower
lip pit, the molars achieved a Class I partnership and the facial soft tissue
profile improved significantly.
