Karpandazic Flap: A Versatile Technique for Lip Reconstruction | Chapter 2 | Key Flaps in Plastic Surgery

This book chapter presents a case report and discussion on the use of the Karapandzic flap for reconstructing a lower lip defect caused by squamous cell carcinoma. The patient presented with progressive swelling involving the left oral commissure and left side of the lower lip, clinically suggestive of malignancy. Histopathological examination confirmed moderately differentiated squamous cell carcinoma. The chapter provides a detailed account of the preoperative assessment, operative procedure, and postoperative period.

The chapter begins by introducing the Karapandzic flap technique and its significance in reconstructive surgery, particularly for lower lip defects. The indications for using this technique are discussed, highlighting its efficacy in achieving successful outcomes.

Detailed intraoperative photographs illustrate the step-by-step operative procedure, starting with wide local resection of the tumour while preserving a safe margin. The lip defect is carefully measured, and the Karapandzic flap is marked and mobilized, ensuring the preservation of neurovascular bundles. Intraoperative photographs depict the reconstruction of the lower lip and commissure using the Karapandzic flap.

The postoperative period is described as uneventful, with satisfactory results observed during follow-up examinations. Frontal and oblique view photographs demonstrate the aesthetic outcomes achieved at six weeks postoperatively.

The discussion section of the chapter provides a comprehensive analysis of the Karapandzic flap technique for lower lip reconstruction. It explores the anatomical basis of the technique, emphasizing the importance of preserving lip subunits and their blood supply. The advantages of the Karapandzic flap, including its ability to maintain lip aesthetics and restore function, are discussed in detail. The limitations of the technique, such as the size and location of defects and the potential for minor asymmetry, are also addressed.

In conclusion, this book chapter offers valuable insights into the use of the Karapandzic flap for reconstructing a lower lip defect caused by squamous cell carcinoma. Through the presentation of a case report, operative details, and a comprehensive discussion, the chapter highlights the effectiveness and benefits of this technique in achieving optimal outcomes in lip reconstruction.
