Study on Remission in Depression and Associated Factors at Different Assessment Times in Primary Care in Chile | Chapter 8 | Highlights on Medicine and Medical Research Vol. 6

 The aim of this study was to find out what factors were linked to remission at 3, 6, 9, and 12 months in depressed adult patients in Chile's primary care (PHC).


Methods: This was a one-year naturalistic study that tracked 297 patients in eight Chilean primary care clinics who were being treated for depression. The international mini-neuropsychiatric interview (MINI), a screening for childhood abuse incidents (CTEs), the Life Experiences Survey, and a relationship violence scale were used to assess patients at first. During the observation period, the patients were assessed using the Hamilton Depression Scale (HDRS). A dichotomous logistic regression and a multivariate Poisson regression were used to test the relationship between the variables analysed and the primary outcome remission (HDRS 7) The level of importance was set at 0.05.

Remission (HDRS 7) ranged from 36.7 percent at three months to 53.9 percent at twelve months. CTEs (Wald X2 = 4,88 Exp B=0.94 CI 0.90- 0.92 p=0.27), psychological comorbidities (Wald X2 = 10.73 Exp B=0.94 CI 0.85- 0.96 p=0.01), suicidal thoughts (Wald X2 = 4,66 Exp B=0.88 CI 0.79- 0.98 p=0.031), and previous treatment depression (Wald X2 = 4.50 Exp B=0.81 CI 0.68- 0.85 p=0.03).

Discussion: At 12 months, almost half of the participants in this study failed to achieve depression remission. Psychiatric comorbidities and CTEs are variables to consider in depressed Chilean patients who have a bad outcome. In PHC, these factors need more attention and a more effective approach. Other interventions aimed at enhancing the understanding of these clinical and psychosocial causes in depressed patients should be integrated into PHC, according to the results of this report.
