Cytology in COVID-19 and Laryngopharyngeal Reflux Disease| Book Publisher International

Introduction: SARS-CoV-2 infection and laryngopharyngeal reflux (LPR) both expose the upper aerodigestive tract to injurious biological agents—viral particles and gastric refluxate, respectively. These exposures can trigger mucosal inflammation, oxidative stress, and genotoxic changes detectable through cytological and immunohistochemical evaluation.

Aim: This study aimed to assess genotoxicity and oxidative DNA damage in exfoliated mucosal cells of individuals with suspected COVID-19 and patients with LPR, and to evaluate epithelial–mesenchymal transition (EMT)–related markers to understand early mucosal remodelling.

Settings and Design: A cross-sectional study conducted at AIIMS, Mangalagiri, between August 2022 and February 2024.

Methods: Exfoliated buccal or pharyngeal mucosal samples were collected from 86 COVID-19-suspected individuals (18–45 years) undergoing RT-PCR testing and from clinically diagnosed LPR patients with healthy controls. Smears were stained using the Papanicolaou technique for cytological analysis, and immunohistochemistry was performed to evaluate oxidative DNA damage (8-OHdG) and EMT markers (E-cadherin, N-cadherin). Micronucleated cells, inflammatory infiltrates, and oxidative marker expression were quantified. Statistical analysis was done using independent t-tests and ANOVA, with significance set at p < 0.05.

Results: COVID-19 RT-PCR–positive subjects showed significantly elevated micronucleated cells, total micronuclei, and inflammatory cell counts compared to RT-PCR–negative individuals, along with intense 8-OHdG expression indicating marked oxidative DNA damage. LPR patients similarly demonstrated increased oxidative stress with strong 8-OHdG staining and higher micronucleated cell frequencies than healthy controls. While E-cadherin expression remained largely preserved, N-cadherin showed moderate upregulation in LPR patients, suggesting an early EMT-related cadherin shift. Both conditions exhibited a reduced epithelial-to-inflammatory cell ratio, reflecting ongoing mucosal injury and remodelling.

Conclusion: Both SARS-CoV-2 infection and chronic laryngopharyngeal reflux exert measurable genotoxic and oxidative effects on mucosal epithelial cells. The increased micronuclei formation, heightened 8-OHdG expression, and early EMT-associated changes indicate genomic instability and mucosal remodelling driven by persistent inflammation and oxidative stress. These findings underscore the importance of early detection and targeted interventions to prevent progression toward long-term mucosal damage and potential malignant transformation.

Pathologic and Immunophenotypic Characterisation in Canine Splenic Lymphoma| Chapter 1 | Medical Science: Updates and Prospects Vol. 2

This chapter provides a comprehensive overview of the pathological and immunophenotypic characterisation of canine splenic lymphoma, focusing on detailed diagnostic approaches and subtyping through integrated analysis of gross, cytological, histopathological, and immunohistochemical features in both biopsy and necropsy specimens.

Background: Lymphoma constitutes the third most common malignant neoplasm, accounting for approximately 7-24% of all canine neoplasms and 83% of all canine hematopoietic malignancies. The aetiology of canine lymphoma (cL) is multifactorial, and environmental factors play a pivotal role.

Aim: This study aimed to characterise canine splenic lymphoma through a comprehensive evaluation of gross, cytological, histopathological, and immunophenotypic features in both biopsy and necropsy specimens, and to distinguish primary splenic lymphomas from splenic involvement in multicentric disease.

Methodology: The study was conducted in the Department of Veterinary Pathology, Madras Veterinary College, India, for one year (June 2011 to June 2012). An observational, descriptive investigation was undertaken on confirmed cases of canine splenic lymphoma submitted for routine diagnostic evaluation. Eight canine cases were diagnosed as splenic lymphoma, comprising six multicentric diffuse large B-cell lymphomas (DLBCL) and two primary splenic B-cell lymphoblastic lymphomas (B-LBL). Specimens were obtained from surgically excised splenic biopsies and post-mortem examinations. Grossly, all spleens exhibited marked splenomegaly, with weights ranging from 120 to 560 g. Patient data revealed a mean age of 8.56 years, with no sex predilection and representation from multiple breeds. Cytological and histopathological classification was performed according to the updated Kiel and World Health Organisation (WHO) criteria. Immunohistochemical analysis employed a Pan T-cell marker (CD3) and a B-cell marker (CD79a) to determine lineage.

Results: The age recorded in this study ranges from 3.5 to 12 years with mean of 8.2 years, and found equal distribution of sex. All cases demonstrated a CD3-negative/CD79a-positive immunophenotype, consistent with high-grade B-cell lymphoma. Six cases represented splenic involvement in multicentric lymphoma, whereas two were classified as primary splenic lymphomas. The cytological study of lymphoma revealed a homogenous clonal population of lymphoblasts with lymphoglandular bodies with variation in multiple nucleoli and scanty basophilic cytoplasm. Histology showed the white and red pulps were completely substituted by B-LBL neoplastic cells. Cells showed anisokaryosis, prominent nuclei, and chromatin margination. Neoplastic cell showing nuclear indentation with a large, single, prominent nucleolus. Histomorphological features corresponded to their cytological classification, and gross lesions were characterised by diffuse enlargement and loss of normal splenic architecture.

Conclusion: The integration of gross pathology, cytology, histopathology, and immunohistochemistry enables accurate subtyping of canine splenic lymphoma, with CD3 and CD79a serving as reliable markers for lineage determination. In this series, high-grade B-cell lymphoma was the predominant subtype, underscoring its clinical relevance and the value of immunophenotyping in routine diagnostic practice. Canine lymphoma is a treatable neoplasm with single-agent or multiple-agent chemotherapy; thus, the early diagnosis ensures longer survivability with appropriate therapy.

Role of Pap Smear in Screening of Infections and Malignancies: An Analytical Study in A Tertiary Care Centre | Chapter 1 | Medical Research and Its Applications Vol. 11

Background: Cervical cancer is the fourth most common cancer in women. In India and other developing countries, cervical cancer is the leading cause of morbidity and mortality. Cancer cervix continues to be the most common genital carcinoma in India accounting for 80% of all female genital malignancies. In India, on an annual basis, about 1,32,000 cases are noted, with each year amounting to 74000 deaths in total, thus every 7th-minute cervical cancer claims the life of one woman. Pre-invasive lesions can spontaneously regress to normal or remain stable for a long period or progress to a higher degree of dysplasia. Cancer of the cervix is preventable if diagnosed at the pre-invasive stage with regular intervals of cytological screening by Papanicolaou (Pap) smears. The aim of the study is to analyse the pap reports in terms of normal findings, infections, premalignant lesions and invasive cancers.

Methods: All women attending the outpatient department of gynaecology at TMMC and RC Moradabad, Uttar Pradesh over a period of 1 year from August 2017-18 presented with obstetrics and with white discharge per vagina were screened for cervical cancer using pap smear. All the smears were reported as per the 2014 Bethesda system. All the collected data were analyzed by using the SPSS tool and descriptive statistics were presented as frequencies and percentages.

Results: Out of 1392, Pap smear reports ASCUS was reported in 27 cases (2%), LSIL in 27 cases (2%), HSIL in 15 cases (1%), malignant cells in 15 cases (1%) and normal including the infection is reported in 1308 cases (94%). Due to changes in lifestyle and changes in demographic profile in developing countries, non-communicable diseases are on the increase as an important health problem that requires effective control programs before, they become involved in their epidemic spread. The most common symptom in this study was white discharge per vagina accounting for 36%.

Conclusions: Early cervical epithelial changes can be identified by a Pap smear test, which is the primary screening test for detection of precancerous cervical intraepithelial neoplasia and the early stage of invasive cervical cancer. The sensitivity of pap as a screening test can be increased with universal screening of all women attending gynecological OPD.

Fine Needle Aspiration Cytology for Early Detection of Cutaneous Metastases: Insights from a Tertiary Care Center | Chapter 13 | Medicine and Medical Research: New Perspectives Vol. 7

Introduction: Skin metastasis can be the presenting symptom of many malignant tumors. These signs occur a long time before the diagnosis of the primary tumors or their recurrence and sometimes after surgery. These initial cutaneous tumors may aid in the diagnosis of primary tumors.

Aim: This study was conducted to identify them and to establish the utility of Fine Needle Aspiration Cytology (FNAC) to diagnose these lesions. As FNAC is a simple, rapid, cost-effective, minimally invasive OPD procedure. No anesthesia or special equipment is needed and it can be practised in a small setup.

Materials and Methods: The study was conducted in a tertiary care center. Clinical history, presenting features, and FNAC findings of the cutaneous lesions were noted. Immunocytochemistry(ICC) and Immunohistochemistry (IHC) from cell block were done when necessary. These findings then corroborated with Histopathological diagnosis.

Results: This study includes the cases of cutaneous metastatic deposits from various primary visceral malignancies, Lung, Gastrointestinal tract, thyroid, ovary etc. at different sites. The sites of cutaneous metastasis are the chest wall, scalp, and anterior abdominal wall, all over the body. The patients underwent preliminary investigations and FNAC. After a diagnosis of neoplastic lesions, the patients were referred to the Radiology, surgery, and radiotherapy department for further management.

Conclusion: Metastatic skin lesions may develop due to lymphatic and blood spread. It may be the first presenting feature of the malignant tumor or its recurrence. This study also proves the efficacy of FNAC in ascertaining the nature of these lesions. The cytological features along with clinical findings and radiological imaging help the early recognition of the primary lesion or identification of recurrence and thus help the clinicians in planning appropriate management.

Exploring Genetic Variations: Polyploid Mutants in Grape Cultivation | Chapter 3 | Current Research Progress in Agricultural Sciences Vol. 8

In order to produce experimental polyploid forms, almost all the methods used are based on the influence of mutagenic agents on actively dividing somatic cells in the selection of grapes. As a result, the obtained genomic mutations are mitotic autotetraploids according to genetic properties. Under the influence of natural mutagenic factors, spontaneous mitotic polyploids usually occur in grapes as a result of the non-disjunction of chromosomes in anaphase.  These days, the selection of grapes to obtain new initial polyploidy material with various inductions of mitotic genomic mutations with the help of colchicine and other mutagens. The duration and complexity of the selection process called for a new method that would increase efficiency and accelerate the selection of polyploid forms of grapes. The study describes the impact of various concentrations of colchicine aqueous solution during inflorescence at the meiosis stage of 3 grape cultivars. A new method for preparing a genomic mutation in the meiotic stage of gametogenesis in the grapes under the influence of colchicine was developed and it is described briefly. The study revealed that mutational efficiency constitutes 2.2-28.0%. This new technique allows for the induction of genomic mutations at the meiosis stage in grapes. The cyto-anatomical and histological analysis of polyploid mutant forms of grapes revealed cytogenetic and histological mutational variation. Genetic analysis of variability is given.

Choosing between FNAC and Core Needle Biopsy: A Battle in Breast Carcinoma Diagnosis | Chapter 5 | Advancement and New Understanding in Medical Science Vol. 6

The present study was conducted in a tertiary care teaching hospital to evaluate the role of FNAC and CNB in diagnosis of suspicious breast cancer by keeping mastectomies- surgical excision (SE) as gold standard. Breast cancer is the commonest female malignancy worldwide. The management of breast carcinoma relies heavily on the diagnostic parameters. Over the years there has been a steady decline in the usage of fine needle aspiration cytology (FNAC) over more popular core needle biopsy (CNB). This was a prospective study done in the department of Pathology of a tertiary care teaching hospital catering to the urban as well as rural population in northern part of India. A total of 60 patients were subjected to simultaneous on site FNAC and CNB. Histopathological correlation (SE) was available in half of the cases only (30 cases). While the Cytological grading was done according to Robinson grading system; the histopathological grading was done by Nottingham Modification of Scarff-Bloom Richardson method. FNAC diagnosed 29 cases whereas CNB diagnosed only 26 cases as malignant with 3 cases signed out as non-diagnostic for malignancy owing to non-sampling from the representative areas. On comparing, FNAC and CNB; agreement was not statistically significant (p=0.14) Comparison of CNB and SE; in 17 cases agreement were noted for both CNB and SE and statistically the values were highly significant (p=0.0001). On comparison of FNAC and SE in 13 cases agreement was noted for both FNAC and SE and the data was statistically significant (p=0.035). It is concluded that FNAC gives 100% results, it is quite reliable investigation and CNB should be done as an adjunct if needed especially for grading of the tumor and for immunohistochemistry.

Evaluation of Cervical Cytology Reports of Women screened by a Non-governmental Organization in Rivers State, Nigeria | Chapter 11 | New Advances in Medicine and Medical Science Vol. 6

According to World Health Organization (WHO) an supposed 570,000 new cases and 311,000 deaths from cervical tumor occurred in 2018 accompanying 80-85% deaths taking place in underdeveloped countries. However, in the developed nations, through organized cervical malignancy screening programs over 80% of mortalities are obviated annually. Across Nigeria, skilled were 14,943 new cases and 10,403 deaths in 2018. The high ailment incidence and humanness rates are largely assign to public unawareness, progressive disease stage presentation and incompetent screening programs containing socio-cultural determinants among possible choice. Cervical cancer is a able to be averted genital tract virulence if precancerous lesions are detected early and immediately treated. The aim of the study search out determine the occurrence of precancerous lesions and highlight on the weak up take of cervical cancer screening. This study was administered over eleven year ending by Preventive Healthcare Initiative *PHI): a (Nongovernmental organization) all along cervical cancer understanding campaigns. Conventional cytological screening was completed activity on two hundred eligible women the one consented to take part in the screening exercise, utilizing the conventional George Papaniolaou procedure. The information was analyzed by plain statistical pattern. Two hundred cytology results of the mothers aged betwixt 25 and 73 years accompanying over 90% within the reproductive ages (40 to 49 age) were analyzed. One hundred and sixty (80%) of the colleagues had never heard of cervical malignancy while 196 (98%) were being screened for the first occasion. One hundred and ninety (85%) of the women were negative for precancerous lesions. Ten (5%) girls had precancerous lesions, [8(4%) had Low Grade Squamous Intra-epithelial Lesion (LGSIL), 1(0.5%) had High Grade Squamous Intra-epithelial Lesions] and 1 (0.5%) had Atypical Squamous Cells of Undetermined Significance (ASCUS).The study found that just 80% of respondents had a weak understanding of cervical cancer, accompanying the majority of ruling class (95%) having sustained their first screening.  There is a need for government and nongovernmental institutions to raise awareness and experience the general folk about cervical cancer stop in order to lower disease mortality.

Determination of Robinson Cytological Grading of Invasive Ductal Carcinoma of Breast| Chapter 5 | Current Overview on Disease and Health Research Vol. 10

Breast abnormal growth in animate being is the most ordinary cause of death in females everywhere, which concede possibility be detected early and acted promptly. In underdeveloped countries, breast carcinoma is still pinpointed using fine-tease aspiration plant structure (FNAC). Instead of simply reporting as favorable or malignant, we concede possibility focus on eliciting the most prognostic facts from cytological smears to help clinicians improve the effects of new chemotherapeutic powers on patient survival. Robinson and others. present a cytological grading order that closely parallels Elston's modification of Scarff Bloom Richardson's histologic grading design. This Retrospective study was done to judge the importance of cytologic evaluating of breast carcinoma and equate it with histological evaluating. The present study was done over a sample proportion of 35 patients for over a span of 12months. 35 Patients the one go through FNAC and mastectomy for carcinoma of conscience were cytologically and Histologically Graded. The results obtained by two together grading procedures were then compared. In the study all were female. On comparison of the cytological and histological grades, 100% cases were grade III, 81% cases were grade II , 72% were grade I two together on cytology and study of animal. The current study demonstrates that a natural cytological grading helps in evaluating the patient's forecast and provides various treatment alternatives depending on the grade.

A Rare Case about Cytomorphology of Primary Adenoid Cystic Carcinoma of Lung| Chapter 3 | Issues and Developments in Medicine and Medical Research Vol.10

Primary adenoid cystic carcinoma of the lung (ACC) is an uncommon tumour that accounts for just 0.04-0.2% of all primary pulmonary malignancies. Primary lung ACC usually strikes between the ages of 40 and 60. It was previously known as bronchial adenoma, which implied a benign glandular neoplasm. It is currently thought to be a low-grade bronchial carcinoma, nevertheless. Pulmonary ACC is a slow-growing tumour that usually develops in the proximal tracheobronchial tree. A 20-year-old woman was presented with a 20-day history of cough without expectoration, weight loss, fever, and anorexia. On Ultrasound Sonography (USG), a well-defined mixed echogenic lesion measuring approximately 10x7 cm was discovered in the left upper zone. Fine Needle Aspiration Cytology was performed under USG guidance. The cytopathology revealed primary adenoid cystic carcinoma of the lung. Fine needle aspiration cytology is a straightforward procedure that can help with pre-operative diagnostics. However, cytological diagnosis of adenoid cystic carcinoma of the lung is difficult. As a result, histolopathological research backs it up.
