Systemic Lupus Erythematosus (SLE): As a Haematological Problem | Chapter 14 | Advancement and New Understanding in Medical Science Vol. 2

 This chapter gives some new observations on Systemic lupus erythematosus (SLE), based on case histories, after studying more than 400 cases in a referral centre. All these were patients who came as diagnostic problems and not with established diagnosis. SLE is a systemic autoimmune disease with multisystem involvement and is associated with significant morbidity and mortality. Genetic, immunological, endocrine, and environmental factors influence the loss of immunological tolerance against self-antigens leading to the formation of pathogenic autoantibodies that cause tissue damage through multiple mechanisms. SLE is the prototype of a systemic autoimmune disorders and is considered a rheumatological disorder, but our observations and studies have established that the commonest initial presentation in SLE is with haematological problems. Or they frequently present with concomitant haematological symptoms, even in the presence of other manifestations. This is only reasonable that the bulk of SLE clinical manifestations must be haematological in nature, because blood and blood vessels collectively carry more antigens than any other organ. The haematological presentations seen are Chronic ITP, Autoimmune Haemolysis, Autoimmune B12 deficiency, Iron deficiency anaemia, Evan’s syndrome, pancytopenia, MDS, refractory anaemias, Pure Red cell Aplasia, leukopenia, Antiphospholipid antibody syndrome, lymphadenopathy with or without Hepatosplenomegaly, acquired clotting factor deficiencies, HELLP syndrome, TTP, myelofibrosis etc. In young females, if they present with one autoimmune disorder, it could be SLE in evolution, even if the tests for SLE are initially negative. If two or more autoimmune disorders are coexisting, it is almost always SLE, irrespective of the age. The criteria used for the diagnosis of SLE also need revision for the simple reason that relying too much on the existing criteria often leads to missing the diagnosis. Our observation was that only very few patients satisfied the ACR criteria for diagnosis. Therefore, it is evident that by several ways SLE qualifies to be called as a haematological disorder or else it should be considered only as an Internal Medicine problem. The fact is that clinical judgment is superior to laboratory diagnosis, but we need both of them. An alternative criterion for the diagnosis of SLE, which we have developed, the Kozhikode Criteria’ is given in the end. The etiology of SLE is largely unknown, but the observations suggest that it could be due to improper diet, lifestyle and probably some problems in their environment.

Differential Roles of the α4β2 and α7 Nicotinic Acetylcholine Receptors in Nicotine Addiction: Implications in Smoking Cessation Medication Development | Chapter 13 | Advancement and New Understanding in Medical Science Vol. 2

Nicotine exerts its reinforcing actions via activating the nicotinic acetylcholine receptors (nAChRs). In tobacco smoking, exposure to the environmental stimuli (cues) previously conditioned to nicotine reinforcement critically contributes to the high relapse rates in abstinent smokers. Research including findings from our own laboratory has suggested critical and differential roles of the two major nAChR subtypes, the α4 β2- and α7-containing receptors that accounts for about 95% of the whole nAChR population in brain, in the process of tobacco smoking addiction development and cue-triggered relapse after a period of abstinence. In specific, our laboratory studies using rat models of smoking and relapse demonstrated that antagonism of the α4 β2 but not α7 subtype effectively reduced on-going nicotine self-administration, indicating attenuation of nicotine reinforcement, whereas, blockade of the α7 but not α4 β2 subtype prevented cue-induced relapse to nicotine-seeking behavior in abstinent subjects. These research observations provide support for the continued effort to test cholinergic agents aiming at the α4 β2 nAChRs for reducing or stopping smoking. However, this work suggests that, in order to prevent smoking relapse in abstinent smokers triggered by exposure to environmental cues, targeting at the α7 nAChRs’ activity would be a promising strategy.

Contamination of Drinking Water by Cyanotoxins in Reservoirs: An Ecological Study Approach | Chapter 12 | Advancement and New Understanding in Medical Science Vol. 2

Objectives: In order to investigate relationship between some cancers (Total cancer cases, primary liver cancer and colorectal cancer cases) occurrence and drinking water consumption from eutrophic reservoirs an ecological study was performed in two Sardinia provinces between 2002-2011 comparing cancer incidence between an exposed area and a reference area. Cyanobacteria are a group of ubiquitous photosynthetic microorganisms existing mainly in freshwater and marine environments.

Indications on the actions to be pursued in the future to protect human health from cyanobacterial blooms exposure.

Methods: The provinces of Sassari and Olbia Tempio are the study area and the Ragusa Province (Sicily) the reference area since its water sources are just groundwaters and springs. Sardinia populations experienced continuous shortage of water resources for thousands of years. To solve this problem, during 1950-1980, the main rivers were dammed constructing 45 reservoirs which rapidly became eutrophic or hypertrophic, with toxic cyanobacterial blooms and a generally poor water quality.

An ecological study has been developed since our work is an observational study characterized by trying to relate exposure and disease at the region and population level. Cancer incidence rates were calculated for the study area and compared to those of the reference area.

Results: Among Sardinian men, for the entire study period risk excesses for cancer of esophagus (Standardized Rate Ratio SRR=3.50), colon-rectum (SRR=1.04), liver (SRR=1.21) and pancreas (SRR=1.24) are observed. Between 2002 and 2011, women showed risk excesses for cancer of esophagus (SRR=3.44) and pancreas during the first (SRR=1.44) and third (SRR=1.67) sub-periods and risk defects for stomach (SRR=0.79) and colon-rectum (SRR=0.92) cancer during the whole study period.

Conclusion: Exposure from cyanobacterial blooms can be considered a risk factor for the onset of cancers of the digestive organs. At a general level, there is the need to plan active water safety strategies on cyanotoxin presence and eutrophic. Prevention and water body remediation can be anyway suggested as preliminary measures to start as soon as possible. Further studies on individual exposure, effective daily intake and chlorination by-product presence are therefore needed to better define the role of this peculiar risk factor; as well as prevention and water body remediation measures.

Primary Fracture Management with Nail and Plate Fixation in Gustilo Anderson 3a n 3b with Antibiotic Cement Beads | Chapter 11 | Advancement and New Understanding in Medical Science Vol. 2

Aim and Objectives: The current study was carried out with the intention of investigating the Primary fracture management with nail and plate fixation in Gustilo Anderson 3a and 3b with antibiotic cement beads.

Materials and Methods: This study was carried out only after receiving the patients' permission to do so, which was required by the institution's Ethical Committee before it could be given the go-ahead for approval. Participants in the research comprised fifty people who had compound tibial shaft fractures of Gustilo Type IIIA and IIIB. These fractures were treated at a tertiary facility using antibiotic-impregnated intramedullary nails.

Results: Majority of patients in our study with compound tibia fractures are belonged to male gender. Left sided fractures were more than right. Majority of fractures in middle and distal third of tibia. There was Superficial infection in 4 (8.0%) and Deep infection among 2 (4.0%) patients. The complications reported were Non-union & Nail breakage among 3 (6.0%) cases and Knee stiffness among 1 (2.0%) case.

Conclusion: According to the findings of this research, the use of intramedullary nails that are coated with gentamicin might be beneficial in the treatment of high-grade open tibia fractures. Despite the fact that the number of patients and infections is too low to draw any firm conclusions, the study did find that there were no severe infections.

Establishing Valid Blood Donor Hematology Standards in Nigeria: Addressing Discrepancies from U.S. Guideline | Chapter 10 | Advancement and New Understanding in Medical Science Vol. 2

Background: Blood transfusion is a lifesaving process and therapy which is required for a wide range of health conditions such as severe anaemia, severe trauma, obstetric complications in pregnancy or parturition and surgical procedures. Adequate selection of a prospective whole blood donor protects his health and safety of the recipient.

Objectives: The main objective of this study was to determine the haematology parameters of apparently healthy prospective whole blood donors.

Participants and Methods: This prospective hospital based study was carried out from August to October 2020 at the blood transfusion unit of the Lagos State University Teaching Hospital (LASUTH), Ikeja, Nigeria. A structured pretested questionnaire was used for data collection. The socio demographic status and the haematology parameters of apparently healthy prospective whole blood donors who tested negative for HIV, hepatitis B and C markers were captured. Obtained data were analysed with the statistical package for the social scientist software version 20.

Results: One hundred male (97.1%) and three female (2.9%) apparently healthy prospective whole blood donors were studied. The median age of study subjects was 30 years. Obtained median haematology parameter values were 13 g/dl, 40%, 4.9/nl and 203.9/nl for haemoglobin concentration, haematocrit, total white cell and platelet counts respectively. The median values for the mean corpuscular haemoglobin concentration (MCHC), mean corpuscular haemoglobin (MCH) and mean corpuscular volume (MCV) of participants were 32.6 g/dl, 27.7 pg and 85.7 fl respectively. Observed prevalence of subnormal haematology parameters for haemoglobin concentration, total white cells, platelets were 12.6%, 25.2%, and 13.6% respectively. Also sub- normal values for MCHC, MCH, MCV were 11.7%, 26.2%, and 16.5% respectively among prospective whole blood donors in this study. No higher than normal haematology parameter values were observed. Median values for erythrocyte sedimentation rate was 8.4 mm/hr. The predominance of the younger people in the blood donor pool may be explained by the fact that the youth have a better knowledge about blood donation, through being vast in digital technology.

Conclusion: A significant percentage of apparently healthy prospective whole blood donors had sub- normal haematology parameters values. Obtained normal values in our study are comparable with local reference range reports from previous studies in Nigeria and other parts of Africa. A health education and awareness pro- gram about the harmless nature of blood donation exercises for the females is critical and should be encouraged and emphasized.

B Flow Assessment of Femoral Artery as an Indicator for the Presence of Coronary Artery Disease in Individuals Undergoing Radionuclide Myocardial Perfusion Scintigraphy for the Evaluation of Chest Pain | Chapter 9 | Advancement and New Understanding in Medical Science Vol. 2

This study highlights about B flow assessment of femoral artery as an indicator for the presence of coronary artery disease in individuals undergoing radionuclide myocardial perfusion scintigraphy for the evaluation of chest pain. In the evaluation of patients with suspected coronary artery disease (CAD), the presence of the superficial femoral artery (SFA) plaque is more informative than a carotid plaque and at least as informative as coronary plaque in the identification of coronary death individuals. A total of 60 emergency room consecutive patients aged 59.2 ± 8.1 years that have been referred to our ambulatory of internal medicine for chest pain evaluation suggestive of ischemic heart disease with a normal or nondiagnostic electrocardiogram were included in the study. We found significant positive correlations between age and SFA plaque score (PS) (P = 0.0084), myocardial ischemia in rest and SFA PS (P <0.0001), and between transient ischemic dilation (TID) and SFA PS (P = 0.0069), too. The TID correlates only with myocardial ischemia in rest (P = 0.0022) and SFA PS (P = 0.0069). The results we got by the receiver operating characteristics (ROC) curve analysis with TID/without TID were the area under curve (0.704, P = 0.0038). The multiple regression analysis showed standardized coefficient b coefficients for SFA PS and TID (3.4577 and 1.9903, P < 0.001 and P = 0.0021), respectively. Proven correlative relationship of SFA atherosclerotic plaques and CAD using these two noninvasive methods gives us the ability to use B-flow as a screening method for triage of patients with chest pain before being sent to the assessment of coronary circulation with radionuclide MPS.

WGS: An Innovative Approach for Management of Tuberculosis | Chapter 8 | Advancement and New Understanding in Medical Science Vol. 2

Whole genome sequencing (WGS) of Mycobacterium tuberculosis is becoming a promising tool for clinical applications for the diagnosis and management of tuberculosis in public health care. Since absence of a consensus and international standards in the analysis and validation of quality controlling of WGS technology, result may lack harmonization and comparability for diagnosis. In this review, the significance of WGS in diagnosis and surveillance is outlined.
