Self-neglect among the Elderly: An Overview of the Concept, Status and Assessment Tools | Chapter 1 | New Ideas Concerning Arts and Social Studies Vol. 2

Background: Self-neglect among the elderly is a widespread public health issue, with its incidence rate increasing year by year, seriously affecting the physical and mental health of the elderly and hindering the realization of "healthy aging". Clarifying the basic connotation of self-neglect among the elderly and developing authoritative assessment tools are of great significance for geriatric research, medical care, and nursing work.

Objective: To understand the concept, current status, and assessment tools of self-neglect among the elderly.

Methods: A literature search was conducted using the keywords "the elder" or "older people" or "old people" + "self-neglect" in English or Chinese in PubMed, Web of Science, CINAHL Plus, Scopus, Embase, PsycINFO, and CNKI over the past 10 years.

Results: Thirty papers were included. Previous literature indicates that the incidence rate of self-neglect among community-dwelling elderly globally ranges from 18.4% to 29.1%. Self-neglect accelerates functional aging in the elderly, increasing their prevalence of diseases, hospitalization rate, suicide rate, and mortality rate.  The connotation of self-neglect among the elderly is complex and multidimensional, with strong cultural dependence, and there is no unified conceptual definition.  Therefore, although assessment tools have been widely developed with many achievements, there are significant differences in the number and connotation of measurement dimensions, as well as the measurement performance of the tools, affecting the comparability and generalizability of assessment results. 

Conclusion: Self-neglect among the elderly has increasingly attracted the attention of the academic community.  However, issues such as the definition of the concept and the scientific development and reasonable selection of assessment tools still need further exploration.

Intrinsic Capacity of the Elderly: Connotation, Measurement, Status and Key Influencing Factors | Chapter 1 | Disease and Health: Research Developments Vol. 6

In the context of "healthy aging", strengthening research on the intrinsic capacities of the elderly, quickly and accurately evaluating them, objectively identifying factors that affect their decline in intrinsic capacities, and improving their intrinsic capacities through optimization measures can minimize the adverse health consequences and contribute to the healthy aging.

Objective: To understand the current research status of the intrinsic capacities of elderly people and provide a basis for targeted and personalized elderly care services.

Methods: A literature search was used for cross-sectional or cohort studies on the connotation, measurement, incidence rate and influencing factors of the internal ability of the elderly published in Chinese and English in the past 10 years in six databases, namely PubMed, Embase, Web of Science, the Cochrane Library, China Biomedical Literature Database and CNKI.

Results: A total of 59 papers were included. The research on the intrinsic capacities of elderly people is still in the stage of exploring theoretical models. The research hotspots include the concept of intrinsic capacities, theoretical support, connotation dimensions, and construction of evaluation systems.

Conclusion: Studying the health status of elderly people from the perspective of intrinsic capacities not only meets the requirements of WHO for healthy aging, but also can build a foundation in the field of geriatric medicine a health security system that better meets the needs of the elderly population. The construction of standardized evaluation tools has to be strengthened, as well as multi-center, large sample longitudinal surveys and randomized controlled trials.

Pyruvate Potentially Superior to NAD+ and Senolytics in Healthy Aging | Chapter 10 | Emerging Trends in Disease and Health Research Vol. 2

The level of NAD+ in cells diminishes with ageing and aging-related illnesses, as is well documented. One of the NAD+ precursors was initially demonstrated with its novel involvement in DNA damage repair in mice several years ago, restoring aged mice to their physical state when they were young. Extensive research in animal models and patients is encouraged as a result of the discovery. NAD+ and its antecedents have long been popular in the nutrition industry. Clearing cellular senescence using senolytics, on the other hand, was shown to sustain multiorgan function (kidney and heart) and extend healthy longevity in mice. Following research in elderly patients with idiopathic pulmonary fibrosis supported the findings. In animal and clinical research, senolytic treatment is increasingly being utilised to treat a variety of illnesses. On an equimolar basis, pyruvate, as a NAD+ substitute and a novel senolytic, may be superior to the present items mentioned in preventing and treating illnesses and ageing. Apart from critical care, pyruvate-enriched fluids, particularly pyruvate oral rehydration salt, could be a novel strategy for illnesses and ageing. Despite the lack of direct evidence that pyruvate, as both a NAD+ source and a senolytic agent, helps healthy ageing, more research is needed to compare NAD+ or senolytics for healthy ageing, specifically on an equimolar basis in effective blood levels. From a clinical standpoint, this review compared NAD+ and Senolytics to sodium pyruvate to explore the recognition of healthy ageing.
