Male Pelvic Congestion and Erectile Dysfunction: Hidden Causes of a Clear Phenomenon in the Young | Chapter 5 | Medical Science: Trends and Innovations Vol. 3

Erectile dysfunction (ED) has an impact on the quality of life of both partners. Erectile function (EF), as a neurovascular phenomenon, is characterized by penile engorgement that results from local arousal-induced release of neuronal and endothelial-derived nitric oxide (NO). ED can arise from arterial etiology, venous leakage, or psychogenic reasons. The study aimed to demonstrate recent environmental reasons behind the rising phenomena of erectile dysfunction among young ages during recent decades. ED can be adequately prevented and improved by the reduction of cardiovascular disease risk factors, regular exercise, weight loss and abstinence from smoking. An apparent role for cytokines in the pathophysiology of ED has been emerging; this substance can induce vascular spasm and affect vascular endothelial function including endothelial-derived nitric oxide production (NO). The association of male pelvic congestion with the frequency of male sexual dysfunction has been documented in literature. As concerns therapeutic modalities, medicines work essentially by mediating or enhancing the effect of NO. ED is lately a common occurrence and its incidence is expected to increase significantly along with the increase in various lifestyle diseases.

Colonic H. pylori strains were found frequently associated with pelvic congestion; H. pylori were considered as a possible underlying etiologic pathology in cases of pelvic pathology in general. The increased mucosal production of inflammatory cytokines of H. pylori could play an integral role in the pathogenesis of ED. The association of the colonic H. pylori strains with pelvic pathology, the role played by the inflammatory cytokines and the therapeutic effect of NO in ED were the scientific reasons to employ colon clear and blood-let out (BLO) cupping therapy in cases of ED. 40 patients with ED positive for colonic H. pylori strains were selected in two age groups; young and older age groups. They were scheduled for three sessions of colon clear, one each month, and a procedure BLO cupping therapy one week after the last colon clear. Marked improvement and most patients recovered their usual EF and sexual satisfaction. It was concluded that employment of combined colon clear and cupping therapy in cases of ED is promising and is not just hypothetical. Upon the basis of this combined traditional natural cure, most of the patients in the study have achieved satisfactory improvement.

Potential of Germanium-Titanium- π Polymer Composites as Functional Textiles in Improving Blood Circulation and Sexual Satisfaction | Chapter 5 | New Advances in Medicine and Medical Science Vol. 5

 Far color of blood (FIR) treatment arises as an interesting approach to enhance the microcirculation structure of the human body. FIR performance can be integrated as an alive textile, that provides FIR exposure for constant use and can be so considered as a potential non-pharmacological therapy in sufferers with minor vascular disorders by frequently reconstructing the blood flow. This chapter aims to show the characteristics and kind of germanium-titanium- π (Ge-Ti- π) textile, manufactured as a patch and shorts, as a promising novel polymer composite for better ancestry circulation of patients accompanying end-stage renal disease going through hemodialysis (HD) and experiencing vasculogenic erectile dysfunction (VED), individually. The immediate and unending effects of the applied Ge-Ti- π fabric have been judged. The Ge-Ti- π textile has existed demonstrated to have 0.93 FIR emissivity, 3.05 g/d strength, and 18.98% extension along with a 51.6% microorganisms reduction and negative fungal growth. The Ge-Ti- π patch considerably reduced appendage numbness/pain (p < 0.001) and pain score on the visual parallel scale (p < 0.001) in patients taking HD. Additionally, the Doppler ultrasound assessment data accompanied a significant augmentation of blood flow in the right hand afterwards 1 week of Ge-Ti- π patch situation (p < 0.041). Furthermore, the Ge-Ti- π underpants situation significantly enhanced the value of sexual function and the average penile ancestry flow velocity after 3 months of the situation in VED patients. Summarily, the results desire that the Ge-Ti- π textile application maybe helpful for victims with impaired ancestry circulation.

Anti-Inflammatory Priming Enhances the Anti-Erectile Dysfunction Efficacy of PDE5 Inhibitors | Chapter 18 | Issues and Developments in Medicine and Medical Research Vol. 3

 Background: Evidence for the role of low-grade systemic inflammation as a co-determinant of erectile dysfunction has been accumulating for some time. Inflammation and disturbance of local microcirculation may potentially prevent PDE5 (phosphodiesterase type 5) inhibitors from working to their full potential. The goal of the idea-generating pilot study described here was to see if reducing such pro-inflammatory contributions to the erectile dysfunctional milieu with the use of a food supplement could improve PDE5 drug efficacy.

Methods: Study Design: Prospective cohort pilot study (14 patients diagnosed with moderate erectile dysfunction) with an end-of-study comparison of final treatment outcomes vs. a random sample of recent history ED (erectile dysfunction) controls from the author's urology ward.

Treatment in a Group Background anti-inflammatory priming for one month (one 1-g tablet once day) with a dietary supplement with known anti-inflammatory and capillary protecting effects, followed by three months of alternating every-other-day treatment with 25-mg sublingual sildenafil or the dietary supplement.

Assessments included a purely exploratory comparison of the prospective cohort's end-of-study EEIF-5 mean scores with a sample of historical controls, as well as a comparison of the overall skewness of the cohort distribution of IIEF-5 (simplified International Index of Erectile Function) scores before and after the treatment period.

The IIEF-5 frequency distribution had a strong left skewness towards the end of the research when compared to the basal score distribution, indicating that reducing background inflammation could be beneficial. Prospective active cohort EEIF-5 mean scores vs. ED historical controls: 20.4 vs. 14.0.

Conclusions: The exploratory comparison of end-of-treatment mean IIEF-5 scores in the active cohort vs historic controls provides preliminary support for the idea that modifying the pro-inflammatory erectile dysfunctional milieu with the anti-inflammatory dietary supplement may have contributed to the overall efficacy of the anti-ED treatment and improved the 3-month efficacy of sublingual sildenafil. Further research is required to corroborate these early findings.

Erectile Dysfunction and Testosterone Deficiency in Young Men: A Retrospective Study | Chapter 10 | Recent Developments in Medicine and Medical Research Vol. 15

 Erectile Dysfunction (ED) is a prevalent sexual problem that affects a growing number of men as they become older, with severe ED affecting 5% of men at age 40 and 15% of men at age 70. The prevalence of ED in young men aged 20 to 40 years ranges from 1.7 percent to 13 percent, according to many studies. Low testosterone is a prevalent cause of ED, but it's not the only one. It's assumed that a normal testosterone level isn't necessary for normal erections, and that adding more testosterone doesn't improve erection function.

The purpose of this research is to find out if young men with ED have hypogonadism. This is a clinic-based retrospective study of young men aged 20 to 40 in South India. A group of 142 males with Premature Ejaculation (PME) and no ED were compared to a group of 269 men with ED, with or without PME, who attended the same clinic. In the PME group, 3% of men had total testosterone levels less than 240 ng/dl, while in the ED group, 17% of men had total testosterone levels less than 240 ng/dl. In the PME group, 22% of men had total testosterone levels less than 360 ng/dl, while in the ED group, 49% of men had total testosterone levels less than 360 ng/dl. PME participants had a mean Total Testosterone (TT) value of 520, while ED participants had a mean TT of 406.ng/dl. All of this is statistically significant. As a result, testosterone insufficiency is not only a common, but also a substantial cause of ED in young men.

Analysing the Determinants of Male Coital Difficulties among Attendees of the Gynae Clinic at a Tertiary Health Center in North-Central Nigeria | Chapter 18 | Current Topics in Medicine and Medical Research Vol. 13

 Background: Male sexual or coital problems are one of the factors that contribute to infertility in couples seeking fertility, as low coital frequency can result. Poor sexual experience or attitudes, low self-esteem, emotional problems, or sexual harassment may all contribute to coital or sexual difficulties.

Aims/Objectives: Over a two-year span, assess the causes of male sexual or coital difficulties among couples attending the BSUTH infertility clinic in Makurdi.

Methodology: This was a cross-sectional analysis involving all males who consented to participate in the infertility clinic at Benue State University Teaching Hospital (BSUTH). The findings were presented in simple proportions after a pretested standardised questionnaire was administered and evaluated with the statistical kit for social sciences (version 23.0).

Male coital problems were caused by a number of causes, according to the findings. Male erectile dysfunction was the most common, accounting for 33 (32.4%), vaginismus 11 (10.8%), penile pain 14 (13.7%), weak female partner answer 8 (7.8%), and extreme dyspareunia 18%. (17.1 percent ).

Conclusion: Coital problems as a result of infrequent sexual exposure are a major cause of infertility among males attending the BSUTH Makurdi infertility clinic. As a result, when evaluating infertility couples, attempts should be made to analyse these factors in order to minimise the impact.
