Compartment Syndrome of the Hand in Multiple Metacarpal Fractures: A Case Report and Review of Literature | Chapter 16 | An Overview of Disease and Health Research Vol. 2

Metacarpal fractures are the most common injuries in the emergency department and account for to17.2% of all adults’ fractures and more than 30% of all hand injuries. The majority of these cases are stable injuries and conservative treatment with closed reduction and immobilisation offers both good clinical and functional outcomes. Compartment syndrome of the hand is an urgent pathological condition which produces irreversible changes to the neurovascular supplies of the hand and may lead to ischemia, permanent loss of hand functions or amputation. The purpose of this study is bimodal: Firstly, to present two cases which presented in the emergency department of our unit, after a traffic accident, with multiple metacarpal fractures and developed acute syndrome of the hand. Based on this condition, a theoretical question should be answered: Do multiple metacarpal fractures increase the risk of acute syndrome of the hand? Secondly, develop a sense of suspicion of this kind of lesion in physicians in the emergency department with a scope to the precocious diagnosis and treatment. While multiple metacarpal fractures may be a responsible factor, the emphasis must be placed on the mechanism and energy of injury, as well as the adequacy of early management, in assessing the risk of compartment syndrome.

Comparison of Accuracy of Gender Identification between Regression Models Derived from Hand, Foot Measurements and Long Bone Measurements in a Sample of Kolhapur Population | Chapter 5 | New Visions in Medicine and Medical Science Vol. 6

 Introduction: Gender identification has been done using the measurements of various body parts such as trunk, vertebral column, limbs, long and short bones, hand, foot, frontal bone, etc.

Aim: The present study aims to compare the accuracy of the regression formula derived to predict gender using data on measurements of foot and hand with the formula derived to predict gender using the length measurements data of the long bones (tibia and ulna).

Methods: The samples are selected randomly from the patients attending outpatient services, the age range of the patients selected is 18-50 years.  (n=1000; 500 males and 500 females). Subjects suffering from any kind of bone deformity were excluded. Vallois method was used to estimate the measurements of hand, foot, tibia, and ulna. Regression formulas were obtained from the hand, foot-long bones measurements; to predict gender, using multiple logistic regression.

Results: Differences between male’s and female’s measurements of hand (P<0.001), long bones (P<0.001), and foot (P<0.001) were significant. The accuracy of the model used to predict gender, which was calculated from the dimensions of hand and foot was 81.5%. The accuracy of the model used to predict gender calculated from the long bone measurement was 78.3%. Surprisingly, dimensions of hand and foot seems to be a better predictor of gender, than long bones (tibia and ulna) length in the Kolhapur population. The major strength of this study is that it is one of the largest studies conducted to identify gender using data on the measurements of hand and foot.

Conclusions: Dimensions of hand and foot are a better predictor (81.5%) of gender vs the length of long bones (tibia and ulna) (78.3%) in the Kolhapur population. The applicability of the methods on populations from other geographic areas should be tested on skeletal samples in which the sex is known, either through documentation, other anthropological methods, or genetic analysis.

A Case Report of Orf Disease in a Greek Island: Diagnostic Tests or Interventions are not Necessary with Scientific Explanation | Chapter 12 | Current Topics in Medicine and Medical Research Vol. 1

The Orf disease, also known as contagious ecthyma, is a zoonotic infection caused by a dermatotropic parapoxvirus that commonly infects small ruminants such as sheep and goats. It is transmitted to humans through contact with an infected animal or fomites. Human infection typically is associated with occupational animal contact. We resently treated a healthy 41-year-old woman who presented a hangnail lesion on the middle finger of her right hand. The lesion was hard cauliflowerlike mass, granulomatous and painful. Surgical debridement and biopsy were performed for further investigation. The Orf disease is usually self-limiting and resolves in 6 – 10 weeks, but complications may occur. The diagnosis may be confirmed by electron microscopy, conventional histopathology or by isolation of the virus by PCR. Early clinical recognition and knowledge of this benign viral condition are vital to avoid unnecessary surgical intervention.
